PRIMARY CARE CLINIC


The Primary Care Clinic includes all the services you receive for your basic, everyday health needs. It is often called the front door to health care - your entry point into the health-care system. It might be a visit to your family doctor, a call to Health Link or an appointment with a therapist. The primary care clinician listens to the patient, makes diagnoses, manages, and screens for other health care problems.

COMMUNITY HEALTH SERVICES


This diverse sector covers a wide range of services, from those targeted at people living with complex health and care needs – such as district nursing and palliative care – to health promotion services – such as school nursing and health visiting.
